May I suggest that you download the application from our website? These are linked to the National Maritime Center so that you get the most current forms. Just go to our application help page. There's a checklist there are a number of elements that go into an application. It's not just the three-page form. I'll go through each one of these and explaining. We're going to go back through them and have a video clip on each one of them to give you a better feel for what is required and how to do it. You have an application with an old statement. You have your transportation worker identification credentials. Furthermore, you have a medical exam. It's a very basic medical exam, but it has to be done. Has to be done by a nurse practitioner physician assistant or an MD. Cannot be done by a chiropractor for example. There's some drug testing requirements. How to prepare Form CG-719s
About Form CG-719s
Form CG-719s, also known as the Small Vessel Sea Service Form, is a document used by the United States Coast Guard (USCG) to record the sea service experience of mariners operating on small vessels. This form is required by mariners seeking to obtain or upgrade their credentials, such as a Merchant Mariner Credential (MMC) or a captain's license. The CG-719s form is applicable to individuals who have served as crew members on small vessels, typically those with less than 100 gross tons or carrying fewer than six passengers for hire. It is mandatory for mariners to submit this form to the USCG as part of their application process for an MMC or any related endorsement. The purpose of the CG-719s is to demonstrate a mariner's sea service experience on small vessels, including details such as vessel information, dates of service, and the capacities in which they served (e.g., captain, mate, deckhand). This form helps the USCG evaluate the mariner's eligibility for the desired credentials and ensures they have obtained the necessary training and experience to operate small vessels safely. Overall, anyone seeking to pursue a career as a mariner in the United States, particularly on small vessels, will need to complete and submit the CG-719s form as part of their application process for obtaining or upgrading their mariner credentials.
